I still say this is fake and that is a CG titty. The way there is no movement no jiggle no shake at all, the under boob shadow looks airbrushed in, there is conspicuous lack of skin detail on her boob. This along with all the quotes from her and the other actor are ambiguous about her actually being topless. I think she was wearing a sports bra or similar that was painted out.
EDIT: Or she did actually go topless but her tits are bunk so they painted better ones over them
